Formerly known as Milestone Foundation, this nonprofit center in Maine supports adults experiencing substance use, mental health challenges, and homelessness. It offers long-term residential treatment, transitional housing, emergency shelter, and mobile outreach. The extended care program uses evidence-based therapies like trauma-informed individual and group counseling. Staff focus on helping men who have not succeeded in shorter programs, using a phased, supportive approach tailored to each person’s needs. Clients also receive case management and recovery-focused services designed to prepare them for reentry into the community.

Clients live in shared rooms within a structured residential campus. Nutritious meals, clean clothes, and showers are included. Transitional housing options include a step-down home with more freedom and sober apartments that support clients as they begin work, school, or outpatient care. The mobile HOME Team offers rides, crisis support, and connections to care throughout Portland and South Portland.
