Aftercare planning at Ascension is a collaborative process involving their care team—physicians, social workers, and nursing staff—as well as the client and, when appropriate, a support person. Tailored discharge plans include referrals to step-down programs or other providers. They also follow up within 72 hours of discharge, helping clients transition smoothly while continuing their healing journey with support and intention.
